Care to Plan is a network of websites for caregivers and physicians who share responsibility for the care of patients with Alzheimer’s and other forms of dementia.
It includes an appraisal tool, developed by the University of Minnesota, School of Nursing, Center on Aging under the direction of Joseph Gaugler, PhD and funded by a grant provided by the Agency for Healthcare Research and Quality. The results of the study are published in The Gerontologist journal published by the Gerontological Society of America.
By answering 20 simple questions, caregivers learn about resources that can help.
It’s free to use and access does not require any form of caregiver or patient identification.
